Child Safety Incident/Allegation Report

• This form must be used to record details of a Child Safety Incident/Allegation.
• If you are a youth member completing this form and need assistance, please speak with a trusted adult.
• If you believe a youth member or yourself is in immediate danger or a life-threatening situation, call Triple Zero (000).
• If a youth member has been harmed or you suspect them of being harmed, a report must be made via Queensland Police Service - Policelink 131 444 or in person at your local Police Station.
• Where appropriate, a report may need to be made to Child Safety Services.
• Any incident reportable to Police must be reported immediately or no later than two hours by calling the State Office on 07 3870 7000 (plus option 9 if after hours). For more information on reporting, please see Child Safe Scouting.
• For incidents that are not Child Safety related, please refer to the Scouts Queensland Incident Report Form.

What happens when I submit a report?

• When your report is submitted, you will receive an on-screen acknowledgement.
• A State Office team member will acknowledge receipt of your report via email during business hours.
• Scouts Queensland will then commence an investigation into your report, unless it has been reported to the Police.
• If the matter has been reported to the Police, they lead the investigation and further updates will come from the Police until their investigation has been completed.
• Once Police involvement has concluded or if police involvement is not warranted, the Chief Commissioner may then initiate an internal investigation or make a determination based on the information already held.
• Any Adult Member that is found to have breached the Scouts Australia's Code of Conduct, will have their role in Scouting reviewed. This may result in the member being counselled, required to undertake further training or may lead to their membership being suspended/terminated.
• Where the alleged offender is a youth member, their membership will be reviewed. As a result, the youth member may receive a warning or be temporarily excluded from activities. Their Unit may receive a recommendation to review their Unit Code of Conduct or arrange a visit from an external authority to educate the Unit. At most, the alleged offender's membership may be suspended/terminated.
